- Release of Liability: As the legal parent or guardian, I release and hold harmless Dance Concepts, LLC, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Dance Concepts, LLC, its owners and operators or in route to or from any of said premises.
- Payment Information: Tuition is due by the second Monday of each month. At this time, electronic checking will be the only option available for monthly tuition payments unless you are paying the year in full. Credit cards may be used for all other types of payment (except tuition). I understand that September and June tuition will be deducted in the last week of September (date TBA). A $45 costume deposit will be due and electronically deducted, by approximately the third Mondayin October (date TBA). Single students are required to purchase 5 recital tickets, families are required to purchase 7 recital tickets. There is a $30.00 returned check charge for any checks returned by the bank. I have either paid my tuition in full or have provided Dance Concepts, LLC my correct banking information to be electronically deducted on or about the second Monday of each month.
- Medical Emergency:The undersigned gives permission to Dance Concepts, LLC, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restictions, or condition and/or declare the paricipant to be in good physical and mental health. Please inform us of your physician's name.
- Summer Enrollment: Tuition for summer is due at the time of enrollment. There are no registration fees currently enrolled students taking summer classes.
- Photograph / Advertisement Waiver: I give Dance Concepts, LLC, my permission to use photos and videos of my child for possible advertisement purposed in brochures, newspaper ads and the studio website.
